A person that is extraordinarily small in size compared to their surroundings. Their small stature could be established against a humongous environment, or with normal sized characters.
If you can distinguish the gender of the small person, tag them as minigirl or miniboy.
For a ordinary sized character and a miniperson in the same image, use size difference. If the larger character is holding the miniperson, use holding mini person or in palm.
